- To request a copy of your motor vehicle, vessel or mobile home record, download and complete form 90510.
- For general questions about your motor vehicle, vessel or mobile home, please contact Customer Service at 850-617-2000 or check your motor vehicle information by entering your vehicle’s identification number (VIN) on our Motor Vehicle Check tool.
- Customer Service Center representatives are available Monday – Friday, 8:00 a.m. – 5:00 p.m., (EST) at 850-617-2000. Please be prepared to provide your driver license number, Social Security number, title or vehicle identification number. Another option available is to can submit an email form by visiting https://flhsmv.service-now.com/csp. A member from customer service will reply within approximately 3 business days.
