  • 82040 MH: Application for Certificate of Mobile Home Title

  • Tax Collectors Mailing List (Form HSMV 83016)
  • Exemption from Florida Sales Tax
  • License Plate Rate Chart (Form HSMV 83140)
  • Florida Insurance Affidavit (Form HSMV 83330)
  • Information on “Discretionary Sales Surtax” to view by county go to the Department of Revenue’s website and click on Form DR-15DSS.
